________________________________. RENTAL/BANQUET RESERVATIONS/DEPOSITS/CANCELLATIONS: MENU SELECTIONS/PAYMENT & GRATUITY POLICIES: Menu selections must be made thirty (30) days prior to the
banquet, along with the $100.00 deposit. Prices will be guaranteed at
that time. Remaining balance is required the day of the event. Cash, checks and credit cards are accepted for payment. PLEASE NOTE: An additional 15% service charge
and 7% Ohio Sales Tax will be added to all banquet invoices. (If tax-exempt,
we must be provided with a tax-exempt certificate prior to invoicing.) Beverages and set-ups for alcoholic beverages must be
purchased from DaLonzo's Restaurant and Banquet Center. DECORATING POLICY: We do not permit the use of rice, confetti, glitter,
birdseed or litter creating items inside the facility or on the grounds. With owner's permission, you may make arrangements to
decorate prior to your event, providing the facility is available at that
time. The person or organization holding the event is responsible for
any damage to DaLonzo's Restaurant and Banquet Center property by any of their
guest. It will be the client's responsibility to pay for any damages that
may occur. DaLonzo's is not responsible for lost, stolen or damaged
articles. DaLonzo's Restaurant and Banquet Center is not responsible for
cancellations or inconveniences due to weather or other circumstances beyond our
control. You may take leftovers with you for any RSVP's which you are
billed for who does not show for your event. DaLonzo's will not provide
containers for leftovers but you are welcome to bring your own and we will
package the food for you. DaLonzo's is not responsible for the refrigeration of
leftover food after it leaves the building. Please be sure that you have
